To access the Claim Form, please enter your ILYM ID (found on the bottom right-hand corner of your CLASS NOTICE AND CLAIM FORM) and your Last Name.

Payments can be made electronically or by USPS mailed check.
If you do not provide an electronic preference, we will mail your payment to the address we have on file. Click to submit your payment preference

If your name or address is different than the one provided on the notice you received, click to submit a change of address.
